In every day and age the only recourse is skepticism. The problem isn't the single journalist or media, it's human nature: likeminded people get together, echo chambers form, nuance is lost, group loyalty overcomes individualism, and The Cause prevails over everything else.

I don't think the numbers were that off, it was just a flawed strategy that even the military knew was nuts but played along. It led to craziness like pacification, where hamlets were cleaned during search and destroy episodes and neatly classed as 'pacified' in the reports to Washington, but nobody told the VC who would return a week later.
