I've been reading about how the RCMP had been consulting "experts" as to whether the cartoon on that sticker constituted ch*ld p*rn (they decided it did not, which surprised me as I assume that common sense has become extinct), but none of the reports showed the sticker. This morning an elderly ex-yakuza boss felt faint, while scooping grounds into his Mr. Coffee filter. He fell to the floor & was too weak to get back up. He lay there for two hours. At some point, he pissed himself. Finally he called a family member & we went to his apartment. I brought a hammer, to bash thru the chainlock on the door. Nothing broken, no pain, he claimed he hadn't fully blacked-out. He was just pale and weak. If there were other symptoms, he hid them-- fearing he'll lose his home & be sent to a nursing home. Much like-- a couple weeks ago-- when he set an egg carton on the stove, then put it in the fridge... resulting in smoke & flames & a firetruck. He claimed that fire was a complete mystery-- spontaneous combustion!-- not a sign he can't take care of himself. I agree with him. I think he should be allowed to fall down & wait for help as much as he wants. No nursing home until he breaks bones or can't stand-up in the 1st place. Then again, I'm not the daughter who has to look after him daily.
I didn't go to the hospital. He pissed himself again in the ambulance. I went home, walked the dog. Dog had a couple elderly falls of his own. Lost balance pooping & nearly sat in it. But the rest of dog's day was fine, loafing.

Sorry to hear about your trouble too. My dad is in a very similar boat. He's currently waiting to get further tests after a GP detected a mild aneurysm on his last check up. His diet has always made Trump's look healthy, so hopefully he takes it seriously.

Secondly, cholesterol tests actually measure the level,of transporters of cholesterol. Of which there are two.
HDL carries excess cholesterol back to liver for reprocessing. This is considered good. HDL is “good cholesterol”.
Normal LDL (pattern A) carries fat and cholesterol to cells for burning/repair. Without it you die. So pattern A is also good.
However. LDL (pattern B) which has been impacted by glucose + genetic disposition becomes ‘fucked up’ (compacts into a dense version) and stops docking with cells. Instead it hangs about (raising cholesterol) until it embed in cell walls, causing inflammation. White blood cells counter attack and plaque is formed leading to heart attacks.
So high LDL either means lots of normal fat transportation OR lots of shit LDL floating around due to demention forcing the building more and more.
Its carbs and genes that fuck you. Not fat. And not “cholesterol”.
Best test. If HDL is high. If lipids are low and if blood sugar traces are normal then high LDL is fine as it’s probably pattern A.

Humans were never meant to consume over about 15 to 20 grams of sugar a day. With the modern western diet it's nearly impossible to get out of breakfast without consuming twice or thrice that. The indigenous people in my locale are plagued with obesity and diabetes, as are the majority of Americans (over half of Americans are pre-diabetic or worse). If you visit the local Walmart or Target or grocery store, the food sections are wall to wall Type II diabetes.

Not quite. Size matters, after all. We all know that oil and water don't mix, so how to transport fat in an aqueous medium, ie blood? Bind the cholesterol to a protein that is hydrophilic, making a lipoprotein. The resulting combination molecules can be large (HDL), small (LDL) and tiny (VLDL). The small and tiny molecules are capable of worming their way into the gaps between endothelial cells that line blood vessels, whereas the large ones are too big to fit. This is why HDL is 'good' cholesterol - it won't clog arteries. It doesn't clean your pipes out, but the more cholesterol is transported as HDL means less being moved around as LDL or VLDL. HDL also has some small effects on vessel wall inflammation, and may inhibit LDL deposition.
On to statins. Much as we learn to become cynical about drug companies, and especially the push to use them for primary prevention (ie treating people who don't have cardiovascular disease with them on the grounds that it is better to prevent than cure), aided by expert panels of docs well compensated for their time and correct opinions by drug companies, these drugs have changed the medical world. Time was when your mid-thirties were the 'heart attack years' and many men died at a young age (œstrogen encourages HDL production over LDL, but women more than catch up after menopause). Between smoking reduction preventing the heart attacks, and statins radically changing the outlook after one has occurred, far, far less premature deaths from heart disease are occurring and consequently cancer is quickly overtaking heart disease as the leading mode of death - simply meaning we have to die of something, and more people are failing to die of heart disease and thus living long enough to get cancer at some later age. There's no doubt if we gave statins to everyone, we could nearly eradicate ischæmic heart disease, peripheral vascular disease and a lot of strokes (not the untreated hypertensive ones, or the berry aneurysms). But we recoil from universal drugging, and also like to deny the manufacturers such obscene profits even if we have to die to do it. It's also true that a small minority of people can't take statins as they get rhabdomyolysis (can be fatal), abnormal liver function (probably not very harmful) and muscle pains (annoying but not dangerous as long as no rhabdomyolysis occurs) - hence the need for regular CPK and LFT testing to identify such patients. We already routinely put diabetics onto statins even if they have no vascular disease, as their risk is high enough that this is justified. Statins primarily lower LDL, and sometimes raise HDL, sometimes not. Getting your LDL as low as possible not only halts deposition of crud in artery walls, it can reverse it, which is remarkable. They can clean your pipes.
In all of this, we have not addressed triglycerides, which are another form of fat in the bloodstream. Type IV hyperlipidæmia is purely a hypertriglyceridæmia, and kills quickly in its serious inherited form. Remember elevated triglycerides are one of the three legs of the 'metabolic syndrome' and thus a problem for nearly all type 2 diabetics. Drug treatments to lower them are not very effective, with statins, fibrates and niacin used, but not as effective as weight loss, exercise and glycæmic control.
Here endeth the lesson, and sorry for the diphthongs. I did learn the simplified norte americano spelling, but am developing curmudgeonly tendencies in my old age.

I'm keeping things simple. I didn't eat that badly before, but apparently not well enough. One of the side effects of immunotherapy is fatigue due to anaemia (the drug lowers H&H quite significantly) so one tends to become sedentary. I was also on oral steroids for an autoimmune retinal disorder which made me eat like Godzilla.
My physio (for surgical scarring and lymphedema management) has helped a great deal, especially as any kind of strength training exhausts me in no time. But I can do lots of moderate cardio. I want to try and manage without statins if possible. Will see what the doc says next week. Here's me numbers, for those curious:

My guess is the doc will strongly push statins, because that's just what doctors do for patients with high LDL and a CVD history. If you want ammunition not to take them, ask for an LDL Particle Test, also called a VAP test. This will measure what proportion of your LDL is more dangerous smaller particles vs relatively benign larger particles. Given that your triglycerides, HDL, and VLDL are all good, I'd say you have a good chance of getting a salutary result on this test.Sulman wrote: ↑ I'm keeping things simple. Thanks for the lesson, very interesting. From my own blood lipid results, I have a discordancy that no doctor has been able to give me any insight on. My results are similar to Sulman's, but even more so. My triglycerides are extremely low, less than half the threshold of concern, and my HDL is in the optimum range. Yet my LDL is on the high side, especially for someone with CVD history. Do you know if this is highly unusual? Every doctor I've asked just shrugs and says I need to focus on the LDL. I was prescribed statins years ago after my stroke and they did keep my LDL's down even though I've only taken them sporadically, but with a recent elevation in my LDL, I've decided to go back to taking them as prescribed at least until I lose 20 lb.screwtape wrote: ↑

Is it inaccurate to identify HDL and LDL as having two seperate jobs, each equally important? Ie one to the liver and the other to the cells?
If that’s accurate, and if your fat readings are ok, is there any inherent issue with pattern A LDL? My understanding is it’s smaller than HDL but still too big to lodge in the blood vessels But above suggests otherwise. I also believe LDL size is on a spectrum and the arbitrary division into A and B is simplistic, but B is shorthand for compacted instances that deviate from the norm due to third party interference.
The topic is all very interesting and not what I heard a year ago from the doctor. He looked at a single cholesterol reading without breakdown and without further discussion wrote out a prescription. Which I carefully put aside while I reconsidered my approach.
