It's a bit puzzling to me because the official narrative is that the responses she was getting drove her out of being a public figure, but the tweeting etc. that she still does is the thing that is most likely to attract a negative reaction. A PhD is tricky enough if you're totally compos mentis, any struggles in that department will only make it more challenging. Not to say that's the case for JM, just saying that in my experience it is a a bit of a rollercoaster and that takes its strain on people. I hope things work out for her.

(Aneris, thanks for the twitter code thing, I simply haven't taken the time to figure it out yet)

Moglia has Crohn disease (formerly "Crohn's disease"). She may have a bias toward the GI tract as a cause of problems. Whether or not that's the case, from the little info I have it seems quite possible that she's not used to looking at medical literature critically (and it also seems quite possible she went off the abstract, not the paper itself, for at least one of them).

The 2 papers she linked are both from 1999, both by gastroenterologists. Each of those should raise questions.

The first one she tweeted I can't access through our medical library (it's in Spanish & from 1999), but the abstract says NOTHING about gender, age, or menstrual history. Bzzzzt (unless she actually read the paper & that info is all in there & appropriate for her purposes, seems unlikely).

The second one she tweeted at least was in premenopausal women, hurray (for her purposes). I can access that paper. It was a small sample (N=19) of women "referred from primary care, internal medicine, or obstetrics and gynecology clinics to the Gastroenterology Service for evaluation of iron-deficiency anemia" so there's a significant bias toward GI causes right there. (Someone who had already figured out that each woman had iron deficiency anemia then figured she needed GI evaluation; they didn't refer the women with iron defiency anemia who seemed to have a non-GI explanation for their anemia). That should be a big ol' flag to STOP RIGHT THERE and not cite this paper as demonstrating GI losses cause anemia more than menstrual losses.

And here's a second big issue with this study: there's no control group.

They divided the group into those who a gynecologist thought had a gyn reason for blood loss & those thought not to, then further into those with GI symptoms and those without, then report that most of the women (with or without symptoms ) had SOME finding on GI evaluation that could cause bleeding (and blood loss and anemia). Not that it DID cause their iron deficiency anemia, or contribute significantly in addition to menstrual losses, but that it COULD. But they didn't do that same careful evaluation in people without iron deficiency anemia.

I found a more recent study (2008) that's larger (N=116) and found a lower rate of GI abnormalities, but still suffers from the referral bias problem.

It's not that I don't think women with iron deficiency anemia can't have GI losses; clearly they can, and it is probably missed more than it should be, but AFAIK there is NOT reliable evidence to suggest that "most" (premenopausal) women with anemia have GI bleeds. (In one case in my own practice, I kept pushing a woman in her early 40's who had severe iron deficiency anemia to have GI evaluation, despite her reluctance; turns out she had advanced colon cancer, but with surgery & chemotherapy she is alive & cancer free, now ~10 years later.)

I think she's right about women being more likely to be dismissed by doctors (but then as a group women also have more contact with the medical system, so men have conditions go undiagnosed, too). The heart attack thing is a bit more complicated (excess mortality in women from heart attacks is explained at least in part by them tending to be older than men with heart attacks, also women's arteries are smaller and less amenable to interventions). Will just leave it at that.

I did not think it was a particularly good idea for him to go, but it is worth noting that he is doing an excellent job of (somewhat) neutrally live-tweeting the proceedings (nobody else there seems to be giving it the blow-by-blow that he is).
