A PhD is tricky enough if you're totally compos mentis, any struggles in that department will only make it more challenging. I do think some of the pressures involved can bring out those sorts of problems in people who were previously just a little neurotic or whatever. Not to say that's the case for JM, just saying that in my experience it is a a bit of a rollercoaster and that takes its strain on people. I hope things work out for her.
